Rehabilitation & Nursing Center of the Rockies is certified by Medicare, Medicaid, and many other insurances on a case by case basis. Our team of health care professionals is led by a licensed administrator who is committed to the health and well being of our residents. Residents and families are encouraged to participate with our staff in the development of each resident’s individualized care plan.

Registered nurses, licensed nurses, and certified nursing assistants work in conjunction with the entire interdisciplinary team to ensure appropriate level of care with areas such as medical needs, assistance with bathing and dressing, assistance with mobility, medication management, and assistance with total personal care.

Our trained staff work closely with area hospice agencies to provide comfort, privacy, and dignity for the patient and family in the final stages of life.

RNCR has an extensive wound care program including an on-site wound care physician who does weekly rounds on all wound care clients. We provide our own wound vac, specialty mattresses, and wound care supplies.

RNCR has a very extensive restorative program that includes programming for modalities such as toileting, dressing, eating, walking and range of motion. These programs run six days per week.

RNCR can offer sub-acute services for feeding tubes, NG tubes, trachea care, short stay rehabilitation care, bariatric care up to 500lbs., respite care, hospice and palliative care and is licensed to provide home health care services for the continuum of care services.

Therapy services are a fundamental piece of our health care program at RNCR. It promotes an expedited recovery allowing our short-term patients to return home sooner. Extended care residents benefit from therapy programs that restore and maintain functional independence and thus improving the quality of their life.

Rehabilitation & Nursing Center of the Rockies has full time professional therapists to serve our residents. Occupational, Physical and Speech Therapy strive to maximize each individual’s functional abilities. We are also the premier skilled nursing facility to offer Anodyne Therapy in Fort Collins.

Activities

Rehabilitation and Nursing Center of the Rockies - Activity Dept offers a meaningful balance of intellectual, creative, physical, spiritual, and social activities to enrich the lives of each and every one of our residents. Activity programs include traditional reading groups, music programs, bingo, guided exercise classes, pet visits, and inter-generational activities while at the same time offering new age pursuits including wine and cheese socials, casino game night, seasonal events, socials and parties. We also provide one on one visits and sensory stimulating activities for our lower functioning residents.
Monthly resident activity committee meetings allow resident input on all planned activities and community outings.
RNCR’s community involvement allows residents the opportunity to participate in outings several times per week such as weekly shopping trips, dining out for lunch, picnics in the park, scenic drives, and even volunteering at the local church. Some of our previous outings have included trips to the Denver Zoo, The Butterfly Pavilion, Celestial Seasonings Tea Tour, Lincoln Center plays and musicals, and fishing trips. RNCR also involves their residents in a Resident Ambassador program to promote resident leadership. Our resident led welcoming committee strives to give newcomers a sense of belonging and encourage new friendships.
Through each of these specialized programs we strive to empower our residents to not only maintain quality of life, but to enhance it.

Social Services

Social Services

Social services is responsible for fostering a climate, policies and routines that enable residents to maximize their individuality, independence, culture and dignity. Social services provides residents with the highest practical level of physical, mental and psycho-social well-being and quality of life. Social services collaborates with physicians, nurses, pharmacists, psychiatrists and psychologists to develop a treatment plan that is individualized to each resident. Residents and their families will be provided with comprehensive pre-admission and discharge assessment and planning to include coordination of in home services upon discharge. Social services also provides coordination of in-house dental, audiology, optometry, and podiatry services, advanced care planning to include behavioral health interventions, dementia/Alzheimer's specific care and ongoing services for residents suffering from mental health concerns to include in- house counseling, psychiatry, interdisciplinary medication management, behavior monitoring and assessment.
